What is Care Navigation?

Whether you are insured or not, our Care Navigators can help you with the following:

  • Answer questions about how a specific preventive services works

  • Understand costs usually associated with specific preventive services

  • Find local clinics where you can get an appointment

  • Find transportation to and from your appointment

How do I receive service?

After you’ve signed up to receive Care Navigation, a Care Navigator will try to contact you at the phone number you provided, during a time period you indicated was convenient. We will attempt to call 3 times.

Once we reach you, we’ll ask questions to better understand your situation and needs. Based on that conversation, we’ll share local resources that might be useful to you. If you’d like, we’ll share this information after the conversation over phone or a mailed letter.

Will I receive medical care or advice?

No. This is an educational service that provides access to generally accepted guidelines for preventive screenings for certain cancers and other conditions. The website will guide you through a series of questions to provide you with relevant recommendations from the guidelines.

This service does not provide medical care or advice or replace a clinician’s judgment. If you have questions or concerns about your health, please talk to a healthcare provider.
